The Protection Plan covers one repair or replacement within 12 months. The cost of the plan depends on what you buy.

There's also a copay when you file a claim, which adds to your total:

We've found the Protection Plan to be especially useful for prescription eyewear since replacing a pair of glasses often costs more than $200. With a total of $149.98, you're still likely to come out ahead. For non-prescription frames, the plan and copay come to $69.98. Since most Ray-Ban non-prescription frames cost more than that, the math works in your favor if you think you'll need a replacement within the year. If you rarely damage your glasses, skipping the plan and saving the $29.99 may be the better option.
